I understand this information will be used to assist my doctor in his/her evaluation of an initial treatment plan or in making modifications to an existing course of treatment.
Integrated Health Solutions is only a secondary means to evaluate your condition. Medical reference ranges are used for your evaluation. In addition, secondary ranges for the average homeostatic individual or “well” patients are used to evaluate and interpret initial changes in your physiology.
I do hereby forever release Integrated Health Solutions, its staff, officers, successors from any and all liability, claims, demands of any kind or nature whatsoever, whether in law, in equity, or otherwise, which arise or may hereafter arise indirectly from the services provided.
