New Patient Information Forms (PDF) Take advantage of eliminating a long wait by preparing your personal/medical history information before you arrive at the office.  We encourage you to either send them to us a week in advance to allow us more time to review your case.   On average, this takes approximately 1 hour to complete and our policy is that we must have this information to review your case. If you can't get this to download send us your email and we'll simply email this to you. Please fax to 866 280 9953 ideally one week before your initial treatment. Often this is not possible. In addition, you may email Dr. Trites.
